Step#2

Fill in your personal information
Under 'Package Information' be sure Take Five! is selected.
Select your metro area from the pull down list.
Apply Rush49614 as a "Coupon Code"
Apply your Voucher id # as Voucher No. (Received from Rush49)
Click the box to agree to the Terms and Conditions
Complete your registration.
